wheel covers,steps and a hitch

I have had a Ford Cube Van of various years since the early 80's.
Just recently I purchased a 1995, 17ft. cube with a powerstroke, (my first diesel, love it) The box has the flat floor which raises the box up a long ways from the ground.

I would like to know if anyone here has installed a trailer hitch, with a reasonable tongue height and how did they manage to do it.

I would also like to install some steps on this van, is there a company that makes a step that attaches at the frame, a double step would be better. I tried running boards before and did not like them.

Now for my final request. What have you guys found to be the best looking wheel simulators? I have purchased a set of Pacific Duallies, but I am not real happy with the way they are attached on the wheel, and they look big and bulky.
