Slipped in 5th?

Hello,
My truck just slipped in 5th gear, I left off, changed lanes and hit the pedal again and it revved up until i let off and then everything was fine.

Any ideas?

OK so this just happened again from a take off around a turn, I believe I was in 1st.

Can someone post a copy of the TSB? Or any other ideas? I have my Ford guy checking also.
