Possible Injector Issue? Advice on next steps?

Every time I start my Truck I get one good "misfire"/squirt/gas puff sound. Thats been going on for about 9 months.

Then about 3 months ago something sounded off... that would happen sometimes when I was driving my car.

The car sounded like it was going to break down last week when it was low on diesel, but after refilling it it seemed like it was doing just fine again. I thought water-in-lines, but I just had the water drained last oil change (but this one tank of diesel was going on 3 months)

Now it sounds worse, it happens every few minutes. Sometimes it sounds even worse (Today it was idling after dropping off a load, and my wife commented that she thought someone slammed on the gas and was stealing it- like it stumpled and caught itself).

Most people I know with injector issues are getting codes, right? I'm getting no codes... so I'm not sure what my diagnostic plan should be... Just wait until it gets even worse? Bring it to a diesel specialist? (closest decent one is a 2 hour trip) Do something to help clean injectors?

I don't drive the truck daily, maximum of a few times/month but the diesel fuel in it now is under 2 weeks old. I also use stanadyne. 188,600 on the clock. Several times a year I'll put thousands of miles on it in a week though.

when was the last time the fuel filters were done? you could have water/rust in the system. ulsd doesn't last near as long as old diesel did. it collects water fairly rapidly. how full was the tank when youhad it sitting 3 months? if it wasn't right full, it could have collected a lot of moisture. did you use an additive?
if they haven't been done in awhile, i'd start with filters. especially considering the driving schedle. you may find an unpleasent surprise up in that secondary fuel bowl. shine a flashlight in there as well, looking for sparkly bits.
should really start by having it scanned with a device capable of reading manufacturer codes tho. could be missing a big piece of the puzzle

Tank was near empty. Fuel filter was just about a year ago, but I only do a few thousand miles a year now that I'm nolonger a competitive driver (we used to haul our race trailer everywhere)

Sounds like it's time to drain the water separater and replace the fuel filter!!
