Smoking like a Train

Have an '04 with 6.0. Going down the road truck begins to smoke heavily, and looses power. Will hardly pull itself. If I let truck set for 15-20 min and start up it will run fine for 20 min to two hours, then the sme thing again. Filter were changed 5K ago. EGR cooler last spring. 120K on truck. Dealer mentioned "sand" in fuel tank and recommened replacement. I didn't see the sand and was skeptical. Could this be an egr related problem?

mine was doin the same thing and it turned out to be a egr valve had it replaced last week ran fine for a few days and it is starti it again i am talking it back to the shop to see what the computer says
