A Little buck or Jerking feeling?

I have a 2006 6.0 with 190K. At around 1700 rpm and holding there is a slight bucking feeling and the needle fluctuates +/- 60 or so and I can't think of what it could be. It happens when the engine is cold warm and hot. I thought I wasn't let it idle long enough before I starting driving but it happens all the time. I am sure someone else has had this problem and knows what that deal is. Thank you in advance.

Most often an injector is failing or displaying as failing. Spool stiction most likely, but it could be a coil or wiring issue. Nozzle issues show up as either over-fuel or under fuel issues, but unless it is gross, you need to bench test for that.
