Valve float/slap???? PICs included

Doing HGs on our 6.0 and each piston seams to have a nice impression of a valve.
They drove it for 5-7k miles with blown HGs but I don't think that's what did it.
I think over revving and high RPMs were the culprit.

It's a stick shift '06
Thoughts???

Valve floating from abuse is my first guess since it's a company truck.

Has it ever had the heads machined? If they decked the heads without trimming the valve stem and reset the seat depth, then the valve would hit the piston. Hard to imagine a shop would make such a serious error, but if they never work on diesels, you never know...

completed in 2011: STC update, new IPR, BPD EGR cooler, oil cooler rebuild, 6.4 banjo bolts, blue spring update, SCT X3, Insight CTS, 58V FICM, CCV Mod, DIY coolant filter (now gone), refinished turbo
2012: ARP headstuds, OEM gaskets, standpipes and dummy plugs
2015: XDP coolant filter

HGs were done 3 years ago by me.
Heads were checked, not decked and HGs were replaced with BOs (this was before everyone realized they were $}{!T and everyone and their dog was using them).
Truck made it 70k miles before blowing, more than most guys that used BOs.

Edit: they were checked not just by me with a straight edge, but miched by our machinists and another machine shop that we use for automotive machining in Amarillo (Carter Machine)
