06 spooling and starting issues

I have an 06 powerstroke that has been pretty good for the most part, however I am starting to notice a few issues popping up:
When I start the car when it is cold out or at high altitudes I get a hard start. It will crank for a while and then upon start up the idle is HORRIBLE!!! It feels like a gas motor with a clogged fuel filter.
When I try to give it some throttle under load after I start it, I get NO turbo spool and it just blows a TON of smoke. I even had the truck falter and die once.
I am still getting 20psi on the dummy gauge, and it never runs hot. Did I smoke my turbo? The truck is under warranty, so if anything goes wrong, I want to get it fixed right. BTW, I currently have 71,000 miles on it.

Pull your codes. Sounds like an injector to me. What color is the smoke? Probably an over fueling or under fuel.

No codes. Black smoke like crazy. Mind you I have the Innovative tuner tuned to the extreme street. I detuned it back to stock and still had the same isseus.

Do you have a egr valve? If you do I would clean that. The egr valve will stick and when you shut the truck off it will reset. So you won't have a code.

check the icp sensor, fuel pressure, injector contribution test and injector buzz test.

Load test your batteries, google ficm check procedure. Do you have a monitor?
