P1378 DTC

Getting the P1378 DTC, causing the truck at random times to loose all power. When truck looses power it smokes white/grey and if you keep your foot into the throttle it turns into black after a few seconds then the truck takes off like nothing was wrong. I tested the FICM and im getting mid 50's running and low 50's with key in the on position. Passenger battery is 13.8v and drivers is 14.0 volts not running, also have high output alternator but can't figure out why this code is coming up.

Any ideas?

cleaned the battery terminals and used electrical spray, issue still present during driving. Checked the wires and didn't see any damage, FICM module is above what is required, possible bad or sticky injector? use diesel kleen and add transmission fluid 1x a month to tank to help lubricate
