what the heck could this sound be??

My buddy has a 2004 6.0 with 180k miles. pretty much bone stock. onlything he has done to it is he is running the newer edge evolution (not shift on fly version) truck has been solid its entire life, never had any issues up until about 10k miles ago it started with some injector issues, rough idle, rough start till warm, injector codes, after warm runs great..

on his way home from a long trip he started hearing a loud rush of air sound when on the throttle.

sounds like one of those accelerator hand dryers in public restrooms. as rpms get higher the sound gets louder.

it will not do it unless its under load. if you put it in neutral and rev engine it does not make any sound.

any help would be appreciated..

Have you checked for a turbo boot leak? They don't have to blow off; a small crack can make a lot of noise and still allow you to build some boost.

Check those boots by removing them. Visual inspections while they are installed can miss quite a bit of area, and the area where they fail the most.
If the boots have a brown/gray look to them, its time to replace them anyway. That color change is caused by the oil from the CCV. The oil breaks down silicone and the oil actually gets pushed through the boot from the inside by boost pressure, making the boot very porous. The oil stays on the outer surface and that brown color is the road grime that stuck to the oil.

Next, the exhaust;
Check the expansion joints on the y-pipe, the connecting points at the manifold, the manifolds to the block and the y-pipe to the turbo connection (check the clamp all the way around with a flashlight and mirror)...basically every connection in the exhaust pre-turbo. If you see soot staining, that is a leak.

Another place to check is the o-ring on the EGR valve. If it is torn, it can cause a noisy leak.

Clutch fan? The thing sounds like it's gonna explode when it kicks on.
