Got Flashed
Well lets start with my symptoms 2004 F250 6.0 all stock. Started with lots of motor noise and rough idle in the cold mornings, and just kept getting worse had to let idle for 45 min. before it would run right. Did fine last winter just started this outta the blue??
Take it to the dealer last thurs. for all the latest calibrations to be flashed, charged 85.00 and away i went. Fixed all my probs, runs like brand new and can jump in start er up on a cold mornin and she runs great. If you are having any of these problems i suggest to get the latest flash.

I had the same problem, though it only took me 15min to idle correctly. Also, no acceleration when I tried to run it cold. Had it flashed at the dealership for $57 and it fixed my problem also.
Glad your truck is running great! Now, I just need to fix the "bucking" problem that I have.... icp sensor, maybe?
