Info needed for a custom tune

I read on here at some point a list of all the info you need to supply the tune writer with so that they can write a tune for you. Can anyone point me in the right direction or happen to know what I will need so I can have what i need when i call?

Ecm strategy
Tuner serial number
And all mods

You put your tire revs per mile directly into the tuner correct?
