Grinding noise and clunk when slowing down.

I have an 03 excursion 6.0 with right about 250,000 miles on it. Today while driving I was coming up to a stop a little above 25 mph and foot off the accelerator and I could hear a grinding noise and then a clunk. It sounded like grinding gears. I don't feel the transmission slipping or anything. The only thing that I have don't is I replaced all my injectors and turbo and reset the computer to relearn the vgt and it has been doing the transmission relearn also. Could this be signs of my transmission on its way out?

hows the fluid level, and when was the last time it was serviced?
