Red face Surge/missing

As I am new to diesel engines I have an issue I can not figure out where to start. I picked up a used Excursion last week.

With a moderate load applied to the engine I get a small surging type feeling along with a deep tone of air being sucked in. When this happens puffs of black smoke comes out. This all happens around 2k RPM range, mostly in the higher gears. When I let off of the long skinny pedal, or apply more the issue goes away. This does not really seam to effect the drivability.

Currently 200K on the clock, I have pulled the EGR valve it was dry along with intake port was dry as well.

I plan on draining the water separator, just to check. I'll be going by the dealership so I might stop by and get all new filters as well.
