Hesitations and loss of power during slow acceleration

My f250 2007 is having hesitations and slight loss of power during easy acceleration. I changed the fuel filters to see if bad fuel. Didn't correct the problem. It seams to be worse when the engine is cold. Any ideas ?

As stated, code would help. Could be something easy like sticking EGR valve.
