Does it run for a second or more when it is cranked ?
Or is it stumbling without idleing ?

I would start with the ficm voltages and fuel pressure.

When I first got my truck I had a similar issue, especially in cold starts. It wouldn't blow THAT much black smoke but it would idle really rough and would stall repeatedly and mashing on the throttle did absolutely nothing. I'd have to restart it about 3 or 4 times and let it idle for about 10+ minutes before I had any sort of throttle response. I ended up replacing the FICM and my problem went away.
