6.0 knocking on acceleration.

I am having ambulance issues. We have a 06 E450 box ambulance with the 6.0, with about 50,000 miles on it. Recently we had it in the shop and had the entire right (I think) bank of injectors replaced (3 out of four were bad), the shop also switched us from ford 15-40 to ford syntheic 5-30, and increased the fuel pressure, I can't remember at which stage. It ran great for a few days. A few days later going to a call, when running 70 plus mph or hard acceleration there was a loud metalic knock, sounds like it comes from the right side I'm not sure though. At just above idle and just as it returns to idle there is a noticeable vibration, there is also tons of black smoke coming from the tailpipe . I took it back to the same shop, they tested it for a week and couldn't pin the noise down to a specific cylinder. They did recreate the noise and vibration.They took all the belts off to rule out any external appliances. On a compression check they found one clyinder lower than the other seven, and think it may be a cracked piston or maybe a bent rod. The shop is a reputable diesel repair shop that I have done business with before. Any help or ideas would be greatly appreciated, since it is an ambulance we don't want to just fix one hole and put it back together like I would my own rig. If we can't figure it out we are looking at nearly a $15,000 being as how difficult it is to change an engine in a van body. Thanks.
