Replace 3 injectors? Got DTC codes

Vehicle- 2003 EXCURSION with 6.0L Powerstroke
Mileage- 111,000
Oil Condition- just replaced with 15w-40 Powerstroke Oil

Symptoms- really rough idle at start up when engine is cold. Engine runs rough for the first 3 minutes or so when driving along.

After about 10 minutes of driving DTC codes of 0270, 0273, 0276 appear. These codes can be cleared (I'm using an Edge console monitor) and the engine seems to run smoothly throughout the day with no other troubles so long as the engine is hot/warm.

Error codes;

0270- Injector 4
0273- Injector 5
0276- Injector 6

These same codes have appeared over several days.

Does anyone have an opinion about my diagnosis?

Is it most likely that the above mentioned injectors are at fault?
(The injectors above are original and haven't been replaced)

What's the likelihood of a chaffed wire harness?
