6.0 Knocking & Clanking- no codes

Hello, Having issues with my 2006 6.0. Has an intake, exhaust, was running SCT tuner but that has been turned off. 94,000 miles, in it's history the turbo has gone, EGR cooler failed and the heads were tested & new gaskets in at 48,000. Injectors failed at 55,000 & 82,000 and in both instances it was repaired under FORD factory warranty.

Just recently repaired FICM through Bulletproof.

The truck has acted funny, under heavy load on hills or towing it would act like it was starving for fuel and hiccup. I have closely monitored the engine & collant temps, they were staying within the 10 degrees.

It was showing codes for "FICM" and "Cyl 4 misbalance" and then after the FICM repair, the codes all went away.

The truck started making this nasty noise while driving it under no load, towing or anything, just idle through the neighborhood.

The shop did a "Cylinder balance test" as well as read the computer and found no codes. Any suggestions? It doesn't sound good.
