I would check your ICP pressures. I just went thru the same thing. It was acting like a bad FICM, but the FICM checked good. I thought the ICP psi looked kinda low but it wasnt bad. Took the cab off for headstuds and the ICP had just started leaking. Replaced it and it has run great since!

Any one cylinder that is dropping off at any time can definitely be caused by a lazy injector. $500 out the door for a new injector iinstalled is very reasonable. I only hope that the injector is coming from Ford or Navistar.
