Improperly installed injector o-ring(s), copper washer(s), improperly torqued/failed injector hold-down(s).

you could in theory pull out each glowplug one at a time on the affected bank until you find which hole makes the bubbles stop. that should tell you which injector is suspect.
That is if you can get the damn harness out without screwing it up an internally failed injector could put compression in the fuel system too. if an injector fails in a manner where compression unseats the pintle air will easily get in the fuel system |

What all those guys said ^ haha. That and general rule of thumb that ive noticed is that if youve got one inj stuck open on a bank, it would be in your best interest to do them all due to the combustion gasses that are able to get into the fuel system and cause the other injectors to score. Remember this is from personal experience and rule of thumb, but I also believe that when under warranty if you have 1 stuck inj on a bank Fords reccomendation is to do all injs on that bank also. Just my .02s

yeah hotline(ahem I mean "prior approval") always tells me to put a bank of injectors in when combustion gas is suspected. never had a problem getting them paid for
