Injector or Tuning Problem?

When I took my heads off this is what my pistons looked like. I've never heard anyone talk about this on a STOCK motor. I have read about the STAR pattern from canned tunes but nothing on a non-tuned motor.

My truck has off an on surged for at least 5years (don't get me going on that) an didn't know if this is the result.

I was also told just before the motor took the dump that my #8injector was bad but I'm not sure an you can see that more then one piston has these marks. I would say all the pass side pistons have it an atleast one on the driver side.

Any idea what caused this and how to fix the problem?
