Found faulty injector with bubble test could it be sleeve?

I did a head gasket job all injectors were fine. Truck smoked and had power loss. I did bubble test and found that when number four cyliner glow plug was removed bubbles stopped. This injector was fine before disasembling. I put new copper washer on the crush looks even and good I see no signs of carbon on the injector or anything and the o rings look good that seal the rail. Could it possibly be coming up thru the injector nozzle meaning it is stuck open. Could it be the sleeve. Whats the best way to tell?
