Could skipping be caused by a blown valve or injector?

2003 w 76k 6.0 skips at 2200-2250 rpm. Could this be caused by an injector or a bad valve?

Does an 03 have an Ficm? Or did that come in a later model year. And would failing a cold crank test make sense if it were an injector?
