04 6.0L Hesitates at take-off and misses under the slightest load/climbing

I've had absolutely no engine problems with this truck over the 80k miles I've put on it...lately however, at highway speeds the engine starts cutting out and starts pumping black smoke...especially when towing a boat, going up an incline or accelerating to pass someone.
If I mash the accelerator to the floor, it will cough and sputter, blow a ton of black smoke and then finally catch and run awesome...but I can't go around town doing 90mph all the time, so I guess I got to get it fixed
The engine idles just fine, but now it is starting to stall a lot on take off...no power at all...when taking off from a dead stop - then, the turbo will finally catch and it will take off fantastically. Anybody have any ideas???

I would try unplugging or cleaning your egr valve. I have seen many-many valves go bad and gum up. It is the valve infront of the oil filter that has a connector that plugs in down and it is stainless.
