Clutch problem

Anyone care to help me guess as to why one of my drivers was able to do this to the clutch on my F550 in 10,000 miles?

How did the throw out bearing look? Pressure plate bolts back out?

Haven't seen pics of the throwout bearing, although it was new 10,000 miles ago. I'm traveling and will see the parts myself on Thursday.
