My uncle converted his 2004 F350 Harley Davidson Crew Cab short bed to a dually about 2 years ago. Looks awesome but is still functionable. I'll be seeing him sometime this weekend. I'll see what he needed to convert it and where he was able to purchase it from if that will help you out at all.
