Yes, the Icon is a complete setup.
You do need the carrier bearing shim kit as well if you have a two piece driveline in the rear.
A dual stabilizer setup is also a huge benefit on the 05+ trucks, IMO.
You can add the shim and stabilizer via the "Customize" button on our site.
To do a 5.5" You'd add a 1" TEI spacer for the front:

In the rear you can either step up to a 6" block or do a One Up Offroad 6" Traction Block...both of which you can do via the Customize page (not sure if you have a 05-07 or 08-09.....here's 05-07):

Or if you have a 05-07 we could do a Icon 5" rear spring.
The Icon 4.5" uses a block in the rear so towing capability is unchanged assuming the tire/wheel combo you use is sufficiently rated for the application.
OUO Traction Blocks in the rear is the ultimate setup overall and the best for towing.
If you do the Icon 5" rears, they are a little softer than stock and thus will sag more under load.

Awesome DC, I really appreciate all the info. Looks like ICON offers all the options and you guys make it easy to customize.
Can get pricey quick! LOL...
But, I guess if a person had to, you could just get the 4.5" kit to start with and add in the other options later.
